Gastric Sleeve Surgery: Post-Operative Dietary Guidelines

What is Gastric Sleeve Surgery?

Gastric sleeve surgery is a restrictive bariatric procedure that reduces the stomach by dividing it vertically into two parts. The remaining functional section takes on a tubular shape, resembling a “sleeve” or “tube.”

This procedure creates a smaller stomach, allowing patients to feel full with less food due to a quicker sensation of satiety. The surgery does not alter the natural digestive process, enabling food to travel to the stomach, fill it by stacking, be held by the gastric valve (pylorus), and digest before moving to the intestines.

How Does Diet Change After Gastric Sleeve Surgery?

In the first few months post-surgery, dietary adjustments, referred to as “weaning,” involve a gradual reintroduction of foods to return to a complete, varied diet. The timeline below is flexible and should be customized with your dietitian or healthcare team:

  • First Month: Semi-liquid and pureed foods.
  • Second Month: Soft, minced foods.
  • After Two Months: A more complete, balanced diet can be reintroduced, gradually incorporating solid foods. Long-term dietary goals should focus on healthy, balanced eating habits and mindful eating practices.

Key Dietary Tips After Surgery

  • Chew Thoroughly and Eat Slowly: Always take small bites and pay attention to food texture. Eating slowly helps you recognize fullness early; ignoring this can lead to discomfort or vomiting.
  • Dedicated Mealtime: Set aside enough time to eat calmly, avoiding distractions. Avoid lying down immediately after eating.
  • Divide Meals: Have three main meals (breakfast, lunch, dinner) and include three snacks (mid-morning, afternoon, and evening).
  • Avoid Drinking During Meals: Drink water between meals, stopping 10 minutes before eating and resuming 30-40 minutes afterward. Aim for 6-8 glasses of calorie-free beverages daily, such as water, tea, or herbal infusions. Avoid carbonated drinks and alcohol.
  • Protein First: Start each main meal with a protein source (meat, fish, etc.) to ensure adequate protein intake for wound healing and immune support.
  • Avoid High-Sugar Foods: Limit foods high in sugar, such as sweets, chocolate, honey, and jams.
  • Reduce High-Fat Foods: Minimize fatty foods (processed meats, aged cheeses, sauces, and elaborate dishes) as they may be harder to digest.
  • Avoid High-Fiber or Hard Foods: These may be difficult to digest and are best avoided.
  • Limit Milk: Initially consume small amounts of milk (such as a small espresso cup), and gradually increase if tolerated. Opt for lactose-free milk if sensitivity occurs.
  • Introduce New Foods Gradually: Test your tolerance by introducing one new food at a time. If any food causes discomfort, try reintroducing it after a few weeks.
  • Continue Supplements: Even with a balanced, varied diet, vitamin and mineral supplements are essential, following your doctor’s instructions.

Physical Activity and Follow-Ups

  • Exercise Regularly: Physical activity is essential to maintain weight loss. Aim for 300 minutes per week of activity, including strength exercises, gradually increasing based on your abilities.
  • Routine Medical Check-ups: Adhere strictly to scheduled follow-ups with your medical team to monitor progress and ensure optimal long-term results.
